A Non-Profit Mobile Health Project Committed to Caring for the Health Needs of Our Community. The Community Care Team can assist people in their own homes. We will work together with the healthcare system to be sure our community members get the best care possible!

What We Do

Our mission is to care for the health needs of our community, and we are seeing an increasing need in the mental health of our area youth. Last year we used our funds to help with the transportation service we provide for clients to get to their medical appointments.

Transportation
